How to refer a patient

Choose a haematologist

Address the referral to a named haematologist. See Our Haematologists for special interests and consulting locations.

Send the referral

Send the referral to the rooms of your chosen haematologist. Contact details for each doctor and location are on their profile and on our Contact page.

Ask your patient to call

Please give your patient the rooms' phone number and ask them to call to book their appointment. Referrals are reviewed by the haematologist, and you receive a letter after every consultation.

What to include

A complete referral lets us triage accurately and often avoids repeat testing before the first appointment.

Patient and referrer

  • Full name, date of birth, address and mobile number
  • Medicare number, and DVA or private health fund details if relevant
  • Interpreter or accessibility needs
  • Your name, practice, provider number and a direct phone number
  • Referral period (GP referrals are valid for 12 months unless you state otherwise; indefinite referrals suit ongoing conditions)

Clinical information

  • The question you would like answered, and how urgent you think it is
  • Relevant history, symptoms and examination findings
  • Current medications, especially anticoagulants, antiplatelets and iron
  • All relevant results with dates, including a full blood examination and film
  • Earlier results where available, so we can see the trend over time
